2567 Statement of Deficiency Text Reporting - Statement of Deficiency Text reports are now available for certain provider types. Use the available filters to generate a table of citations with associated providers and surveys. In the "Statement of Deficiencies Report" column, you can view deficiency texts in either PDF or web page format.
01/21/2026 - Several nursing home reports now utilize iQIES as their primary data source. iQIES, a cloud-based platform, is designed to replace the QIES system. The specific nursing home reports are listed below.
04/28/2025 - The Resources page has been improved with a redesign. As part of the changes, the
Approved Transplant Program List has been moved to the Hospitals section under Provider Reports in the side navigation bar.
03/28/2025 - CMS no longer requires a warning banner when entering the QCOR website. QCOR URLs can now be directly linked to.
Warning: Important System Migration Notice
Provider and survey information for Nursing Homes is partially migrated using the most up-to-date information from iQIES. All other nursing home reports use QIES data complete through July 10, 2025.
Migrated Reports:
Due to ongoing system migrations, provider and survey information for the following provider types does not contain the latest information:
- ICF-IIDs - complete only through March 19, 2026
- CORFs & OPTs - complete only through March 12, 2026
- Dialysis Facilities (ESRD) & X-ray Suppliers - complete only through February 23, 2026
- Organ Procurement Organizations (OPO) - complete only through December 4, 2025
- Hospices - complete only through September 29, 2022*
- Ambulatory Surgical Centers (ASC) - complete only through May 9, 2022
- Home Health Agencies (HHA) - complete only through May 19, 2021
*The
Hospice Survey Reports has data to December 2025.

CMS is working to update the QCOR website during the system migration.
