This STANDARD is not met as evidenced by: Based on record review and interview the hospital failed to ensure the Home Health Agencies requested to be listed as available before placing them on a list to present to patients making post-acute care arrangements. Findings: Review of the documentation provided and medical records revealed a list of Home Health Agencies was provided to each patient prior to discharge to assist the patient in finding post-acute care. A request was made for the hospital to provide evidence that each agency listed had asked to be listed or had approved prior to being placed on the list. In interview on 04/09/2026 at 1:25 PM, S1NSM verified the providers did not ask to be listed. |
